Annual Policy renewal date is July 1st.

 

This plan also provides out of network benefits for exams, frames and lenses only-  Member pays for services and materials up front and submits a Reimbursement Form to receive allowances less the applicable co-pay.

 

Discounts (in network only) on upgrades, options and add-ons is available at no additional cost and with unlimited use of the benefits.  Contact Lens purchases are available at a 20% discount.  Disposable contact lenses are available at a 10% discount.

 

**LASIK correction procedures are provided by TruVision (1-866-486-2020) and TLC Vision (1-877-752-6852) .
